Abstract
Abstract - Patient safety is a fundamental pillar of healthcare services, particularly in high-risk environments such as the operating room. This study aimed to identify factors significantly associated with patient safety in the operating room at Wates Husada Hospital, Gresik. This study employed a quantitative design with a cross-sectional approach. The study sample comprised 52 patients undergoing surgical procedures in August 2025, selected via Simple Random Sampling. Data on independent variables (type of surgery, comorbidities, nutritional status, provision of pre-operative information, duration of surgery) and the dependent variable (patient safety incidents) were collected from medical records. Data analysis was performed using the Chi-Square test. The results indicated a very low rate of Adverse Events (AE) (2%). Nevertheless, statistical analysis revealed a significant association between the type of surgery (p=0.050), patient comorbidities (p=0.005), nutritional status (p=0.007), provision of pre-operative information (p=0.038), and duration of surgery (p=0.010) with the occurrence of AEs. The low AE rate indicates a robust system; however, the statistical significance suggests that the incidents that occurred were not random but were concentrated among high-risk patients. Type of surgery, comorbidities, nutritional status, information, and duration of surgery are important predictors. It is recommended that the hospital strengthen specific safety protocols for high-risk patients. Keywords: Patient Safety, Operating Room, Risk Factors, Adverse Events.
