Abstract
ABSTRACT Hiperbilirubinemia during the transitional period usually improved quickly, but severe cases required phototherapy, which was the main intervention yet had the potential to cause stress and affect the physiology and recovery of neonates. This study aimed to determine the relationship between phototherapy and bilirubin levels as well as stress levels in neonates with hiperbilirubinemia in the Neonatal Room of RSU Al-Islam H.M. Mawardi. The study employed an analytical correlation design with a cross-sectional approach. The population consisted of all neonates with hiperbilirubinemia (bilirubin level ≥5 mg/dl) in the Neonatal Room of RSU Al-Islam H.M. Mawardi in February 2026, totaling 37 neonates. Accidental sampling was used, resulting in a sample of 33 neonates. The instruments used were an observation sheet and the Neonatal Infant Stressor Scale (NISS). Data were analyzed using the Spearman Rho test. The results showed that phototherapy was administered for 1×24 hours and 2×24 hours in almost half of the cases, each consisting of 11 neonates (33.3%). Nearly half of the neonates experienced hiperbilirubinemia grade 5, totaling 14 neonates (42.4%), and the majority showed moderate stress, totaling 19 neonates (57.6%). The Spearman Rho test indicated no significant relationship between phototherapy and bilirubin levels (p=0.417), but a significant relationship was found between phototherapy and neonatal stress levels (p=0.021) in neonates with hiperbilirubinemia in the Neonatal Room of RSU Al-Islam H.M. Mawardi. Phototherapy was effective as the main intervention for hiperbilirubinemia, but its duration did not always significantly reduce bilirubin levels, while the intensity of the therapy affected neonatal stress levels. Nurses were advised to adjust the duration of phototherapy according to the clinical condition of each neonate and implement comfort strategies to reduce stress during care.
