Abstract

ABSTRACT Perineal rupture is one of the causes of postpartum hemorrhage in both first and subsequent deliveries. Perineal rupture affects 83% of women who deliver vaginally. Perineal rupture causes dysfunction of the female reproductive organs, such as bleeding and infection, which can lead to death. The purpose of this study was to determine factors associated with the degree of perineal tear in women giving birth in the delivery room at Sakinah Islamic Hospital in Mojokerto in 2025. The study design was an observational analytical cross-sectional study. The population was 523 women who gave birth vaginally with perineal rupture, and a sample size of 227 was selected using simple random sampling. Statistical analysis used the Spearman's rho test with an α value of <0.05. The majority of respondents were multiparous (117 respondents (51.5%). Almost all respondents had delivered normal newborns (221 respondents (97.4%). Almost all respondents did not experience precipitous labor (226 respondents (99.6%). Half of the respondents, or 115 respondents (50.7%), experienced second-degree perineal tears. The statistical test results showed a significance value of <0.001 <0.05, indicating a relationship between parity and the degree of perineal rupture in the delivery room at Sakinah Islamic Hospital, Mojokerto Regency in 2025. Parity and birth weight influence perineal tears due to the birth canal through which the baby's head passes, the perineal muscles not yet fully developed, excessive pushing, edema and fragility of the perineum, and operative delivery. Health workers, especially midwives, are expected to be able to predict the probability of parity, birth weight, and precipitous labor on the incidence of perineal rupture, so that appropriate preventive measures and management can be implemented.
