Abstract

Abstract - Hazard Observation Card is an administrative hazard control program. The study aims to determine the relationship between the Hazard Observation Card program and near misses at Petronas Lamongan Shorebase. Quantitative research with cross-sectional design. The population of the study were employees working at Petronas Lamongan Shorebase and Sub Contractors totaling 50 people. The sample consisted of 45 respondents. Analyzed using the chi square test. The results of the study showed that most (62.2%) respondents implemented the Hazard Observation Card program well. Most respondents (53.3%) experienced high near misses. The results of the analysis showed that there was a relationship between the Hazard Observation Card Program and Near Misses at Petronas Lamongan Shorebase, Lamongan Regency with a p value of 0.002 <0.005, OR = 0.119, 95% CI = 0.027-0.516. The Hazard Observation Card program can identify unsafe conditions or behaviors in the work environment that have the potential to cause near misses. It is expected that workers will be able to implement the Hazard Observation Card program to increase safety awareness and prevent accidents and reduce their consequences.
