Abstract
ABSTRACT Chronic diseases represent a major global health burden, affecting both physical conditions and overall quality of life. To address this issue, BPJS Kesehatan introduced the Chronic Disease Management Program (Prolanis) to ensure sustainable health services for participants with chronic illnesses. However, Prolanis achievement in East Java declined to 49.65% in 2023 and further decreased to 42.46% in 2024. This study aimed to analyze the influence of knowledge, the role of health workers, and family support on the active utilization of Prolanis in the working area of Balen Health Center, Bojonegoro Regency. A cross-sectional design was applied with simple random sampling involving 154 respondents. Data were collected using researcher-designed questionnaires completed by participants. Statistical analysis employed chi-square tests at a significance level of α = 0.05, followed by logistic regression. Findings indicated that 50.6% of respondents had good knowledge, 51.3% perceived the role of health workers positively, 53.2% reported positive family support, and 53.9% actively utilized Prolanis. Chi-square results demonstrated significant associations between knowledge (p = 0.000), the role of health workers (p = 0.000), and family support (p = 0.000) with Prolanis utilization. Logistic regression revealed knowledge as the strongest predictor (OR = 31.899), followed by the role of health workers (OR = 9.121). In conclusion, knowledge, health worker involvement, and family support significantly influence Prolanis participation. Strengthening these factors is expected to enhance patient motivation, encourage consistent engagement in Prolanis activities, and ultimately improve health outcomes and quality of life Keywords: Knowledge, Perception, Support, Family, Prolanis
