Abstract

Abstract - Self-efficacy was an individual's belief in their ability to perform health behaviors and played an important role in hypertension management. Adherence to medication and a low-salt diet was an important component of hypertension management. This study aimed to analyze the relationship between self-efficacy and medication adherence and dietary adherence among patients with hypertension. This study used a cross-sectional design. The study population consisted of hypertensive outpatients at Reksa Waluya Hospital, Mojokerto City, from November to December 2024. A total of 80 respondents were selected using simple random sampling. Self-efficacy was measured using the Medication Adherence Self-Efficacy Scale-Revised (MASES-R), medication adherence was measured using the Morisky Medication Adherence Scale-8 (MMAS-8), and dietary adherence was measured using a dietary adherence questionnaire. Data were analyzed using the Spearman's Rho correlation test. A total of 69 respondents (86.3%) had moderate to high medication adherence, while 68 respondents (85.0%) adhered to the recommended diet. The analysis showed significant relationships between self-efficacy and medication adherence (p < 0.001; r = 0.570) and between self-efficacy and dietary adherence (p < 0.001; r = 0.384). The relationship between self-efficacy and medication adherence was positive with moderate strength, while the relationship between self-efficacy and dietary adherence was positive with low strength. These findings indicated that higher levels of self-efficacy were associated with a greater tendency to adhere to medication and dietary recommendations. Self-efficacy was an important psychological factor supporting hypertension management behaviors. Strengthening self-efficacy through education, goal setting, identification of barriers, and continuous support needed to be integrated into nursing care to improve patients' ability to maintain adherence to medication and dietary recommendations.
