Abstract
ABSTRACT Labor that fails to progress can lead to impaired uteroplacental perfusion, which may result in fetal hypoxia and ultimately require operative delivery. This study aimed to determine the relationship between the duration of the first stage of labor and fetal distress with the incidence of cesarean section at Prima Husada Sukorejo Hospital in 2025. This research employed an analytical design with a cross-sectional approach and used a simple random sampling technique. Data were obtained from medical records and analyzed using the chi-square test, with a total sample of 92 in January 2026. The results showed that prolonged first-stage labor occurred in 59.8% of cases, most respondents experienced fetal distress (63.0%), and 66.3% underwent delivery by cesarean section. Statistical analysis revealed that the relationship between the duration of the first stage of labor and cesarean section had a p-value of 0.001 (p < 0.05), and the relationship between fetal distress and cesarean section had a p-value of 0.000 (p < 0.05), indicating a significant association between prolonged first-stage labor and fetal distress with the incidence of cesarean section at Prima Husada Sukorejo Hospital in 2025. This study is expected to emphasize the importance of optimal monitoring of labor progress and fetal well-being to prevent complications and reduce the rate of cesarean sections without clear medical indications. Keywords: Duration of the first stage of labor, fetal distress, sectio caesarea incidence.
