Abstract
ABSTRACT Maternal Mortality Rate (MMR) is one indicator to see the success of maternal health efforts. The results of the data from the Trenggalek Regency Health Office that maternal deaths occur due to hypertension. Continuity Of Care midwifery care is carried out completely and continuously on Ny. "D" 23 years old from 40 weeks of pregnancy, childbirth, postpartum, neonates and family planning. This midwifery care was carried out on April 21, 2021 in the working area of PMB Atik Andriani, S.ST. Implementation is carried out with a midwifery management approach and documentation using SOAP. During the pregnancy visit, the mother had no complaints. In childbirth, the complaints felt by the mother are physiological. During the puerperium, she complained that her feet were swollen. The care given was not to wear the stagen too tightly, and to elevate her legs while sleeping, and to drink lots of water. During the neonatal visit there were no complaints. Mother's care for family planning, the mother uses a contraceptive injection for 3 months. The management of midwifery care given to mothers has complied with the health procedures, namely wearing masks, washing hands, using hand sanitizer before carrying out actions during visits, and wearing complete PPE when assisting childbirth, the mother has carried out rapid before. It is hoped that the client can apply the counseling that has been given during midwifery care and apply the prokes so that the condition of the mother and baby remains good and can prevent complications and death.
